Fitness for Duty Evaluation FAQs

  • We operate in-person and via telehealth in 42 states. Telehealth is typically preferred, but please reach out for a list of current in-person testing locations. Currently we do NOT have any scheduling available for Hawaii, Alaska, California, New Mexico, Louisiana, Iowa, New York, and Massachusetts.

  • Our consultant psychologist will ultimately determine, based on clinical relevance, which battery of tests should be administered, but frequently we utilize any of the following assessments:

    • Personality Tests (MMPI2/3 and Personality Assessment Inventory)

    • Cognitive and Neuropsychological Tests (WAIS-5, Trails A & B, Stroop)

    • Symptom Validity Testing (TOMM, SIRS, VIP, MSVT, Rey 15)

    • Risk Assessment (HCR-20, LS/CMI, PCL-R)

    Reports are always tailored to the unique circumstances of a given payor’s needs

  • We will initially set up a consultation meeting to discuss your exact needs. You will receive an informational packet and a referral form. Once we receive the referral, we will schedule the initial meeting with one of our psychologists.

  • Each evaluation is unique, and length will depend on what is requested and what battery of tests are needed. On average, the interview itself will take 4-6 hours with another 2-3 hours dedicated for report writing and record review.

  • Our typical turn-around is within 10 business days. Express delivery is available for an additional fee.

  • Each provider we work with is a doctorate level licensed psychologist who has undergone specialty training in forensics or in fitness for duty evaluations. Senior forensic and clinical psychologists lead our team and provide expert review of cases.

  • Standard FFDE - $2,000
    Complex FFDE* - $3,500
    Formal Violence Risk Assessment - $1,500
    Emergency/Expedited FFDE (48–72hr turnaround) - $500/occurrence
    Follow-up Addendum Report - $500/occurrence
    Court/Testimony Prep (if subpoenaed) - $350/hr

    - custom rates are available for unique or specialty catered requests

    *Complex assessment may be warranted depending on specific needs of the employer, or is automatically included for any of the following vocations: healthcare, military, police, first responders, EMS, firefighters, aviation

Social Security Disability Evaluation FAQs

  • We currently operate in-person in 17 states and counting. Telehealth services are offered in addition whenever needed. Our main office is located in Providence Forge, Virginia.

  • We schedule directly with Disability Determination Services at the state-level.

  • Please check our Indeed job postings for current roles, or feel free to reach out directly here.

  • We aim to have all reports reviewed and submitted within 7 days. We can often get reports submitted much sooner than this.

  • Each provider we contract with is an experienced clinician in their given field. Our contractors are given specialty training on disability evaluation examinations in addition to their clinical background.

  • We have multiple full-time staff members who are responsible for coordinating scheduling, reviewing reports, submitting reports and invoices, and for contacting claimants to remind them of appointments. Our team is a one-stop shop for anything related to SSA disability evaluations.
